RTAD-227 Respiratory Clinical Care Practice III

Credits 5 / 15 Contact Hours
Pre-requisite: Division signature required; Program eligibility requirements; Placement into College Level Reading, completion of ELAP-120 with a 2.0 or higher, or successful completion of ACRD-090, ACRD-091 or ACRD-092. Co-requisite: RTAD-221, RTAD-222 and RTAD-225. Pre- or Co-requisite: RTAD-150, RTAD-154 and RTAD-155.
Provides a more in-depth exposure to the practice of critical care and specialty care. Directed clinical practice includes adult, pediatric and neonatal intensive care, home care, pulmonary rehabilitation, and diagnostic testing.

Course Outcomes
Upon completion of this course, the respiratory therapy student will achieve the following objectives as evaluated by written examination and clinical simulation. An overall score of at least 75% is required for satisfactory course completion.
Topic: Advanced Mechanical Ventilation Objectives:
1. Apply age specific mechanical ventilation.
2. List the indications and hazards of continuous mechanical ventilation.
3. Perform the tasks necessary to initiate mechanical ventilation for patient use.
4. Conduct therapeutic procedures to achieve adequate spontaneous and artificial ventilation.
5. Conduct therapeutic procedures to achieve adequate arterial and tissue oxygenation.
6. Evaluate and monitor patient’s response to continuous mechanical ventilation procedures.
7 Make necessary modifications during continuous mechanical ventilation based on patient’s response.
8. Recommend modifications in the respiratory care plan based on the patient’s response.
9. Perform the tasks necessary to discontinue mechanical ventilation.
10. Perform critical care diagnostic procedures and interpret data.
11. Apply NIPPV

Topic: Neonatal & Pediatric Care Objectives:
1. Respond to respiratory distress/respiratory failure in the neonatal and pediatric population.
2. Demonstrate set up and monitoring of clinic specific neonatal/pediatric mechanical ventilators.
3. List and explain the conditions that indicate the use of continuous mechanical ventilation.
4. List and describe the potential hazards associated with continuous mechanical ventilation.
5. Recommend therapeutic modification to ensure oxygenation and ventilation for infant receiving mechanical ventilation.
6. Demonstrate the use of blenders in the neonatal intensive care units.
7. List and explain the conditions that indicate the use of nasal / mask CPAP.
8. List and describe the potential hazards of nasal / mask CPAP.
9. Administer and monitor nasal / mask CPAP.
10. Identify the need and describe the technique of surfactant administration.
11. Explain the purpose and describe the technique of transillumination.
12. Observe additional neonatal/pediatric interventions (Neopuff, surgical intervention, etc.).
Topic: Cardiac Diagnostics, Surgical, and Physician Interactions Objectives:
1. Demonstrate appropriate placement of electrocardiogram leads for 12 lead ECG.
2. Perform ECG testing and processing of electrocardiogram.
3. Identify ECG-arrhythmias and treatment.
4. Identify and contrast various differential diagnostic cardiac testing procedures.
5. Identify and if applicable demonstrate the procedure for oral intubation.
6. Identify key points during physician/patient interaction in the acute care, extended care, and or the physician office setting.
Topic: Advanced PFT’s Objectives:
1. Demonstrate and evaluate spirometry before and or after bronchodilator.
2. Differentiate between spirometry and full PFT testing.
3. Demonstrate and evaluate diffusing capacity tests.
4. Demonstrate and evaluate FRC by plethysmography.
5. Demonstrate and evaluate flow volume loop tests.
6. Interpret and classify PFT finding (obstructive, restrictive, diffusion impairment, bronchodilator responsiveness).
Topic: Advance Hemodynamic Monitoring Objectives:
1. Observe hemodynamic monitoring interventions.
2. Demonstrate ABG sampling from the radial artery.

Topic: Sleep Objectives:
1. Describe the patient population requiring sleep studies.

3. Identify the hazards associated with a sleep disorder.
4. Participate in gathering “sleep hygiene” history of the patient prior to study.
5. Describe the process of patient preparation for a sleep study.
6. Describe and explain the significance of the parameters monitored during a sleep study.
7. Describe trouble-shooting techniques during sleep testing.

Topic: Emergency Care (ER) optional Objectives:
1. Identify the role of the respiratory therapist in emergency care.
